Employee Evaluation

I seem to recall someone sharing the form they use on here for their evaluations. But, I can not seem to find it with the search tool. We are about to do our Employee Evaluations and I was wanting to update our forms with some questions to foster some positive conversation about the goals of the employees and the changes it will take to get there. As well as us discussing the culture and goals of the company and what we need to do as a team to make those changes happen as well.

So, what are some of the questions you guys would suggest using?

I don't think it has been posted here, but Paul Downs has referred to his evaluation process in several posts to his invaluable blog at "You're the Boss" on the New York Times website. Definitely worth a few minutes searching.
