Fish eye occurring when using Water Base Pigmented finish

Ok I am kind of baffled about this one. I am trying to eliminate a contaminate
type issue in the coating by replacing it. Then I tried swapping out the the equipment for another. Last I sprayed over a different type of material to make sure nothing was on the surface or bleeding through.
Question is what have you ever encountered such an frustrating issue?
I have found that even with all my years of experience I am still presented with crazy challenges. This is one has got me stumped.
Was there anything that could be done to minimize the fisheye reaction that you have tried in the past and has worked to get the project out?
Your inputs would be great.

Not enough information. We would need products used and finish schedule to help thoroughly.
I can say what has solved this using Chemcraft WB products is to not just sand with 320 but go over with super fine sanding sponge. That was after trying all other typical solutions trying to resolve the surfaced tension and or contamination issues. Happened a few times. This always solves it.
